Overview
We are committed to maximizing the independence of older adults. We strive to ensure that the highest quality and most cost-effective services are secured on your behalf.

We help you to obtain home care, to select an Adult Day Program, Assisted Living Community or Nursing Home, as appropriate.

We guide you through the Medicaid process from start to finish whether at home, in assisted living or in a nursing home.

Each person’s situation is unique. There are times when family members or friends find it increasingly difficult to manage the responsibilities of caregiving. It is so important to avoid naturally occurring caregiver “burnout”, when the caregiver feels exhausted, overwhelmed, irritable, discouraged and susceptible to illness.

We will prepare you in advance of our initial consultation. We will meet to review your family’s health care and financial needs and develop strategies tailored to achieve your goals.

We will be available to answer your questions. Our goal is to provide the security of knowing that we will address your issues together. We would like to give you the support you are looking for.

You will not be doing this alone.
Services
We can provide information and referrals to legal, financial, health care, support groups and other social service agencies and professionals who specialize in working with older adults and their families.

Care Coordination

We will:

  • Help you to obtain the appropriate assistance so your loved one can remain at home
  • Home Care, downsizing and organizing
  • Assist with selecting the right model (Social or Medical) Adult Day Program
  • Assist with the selection of an Assisted Living Community that will meet your needs and budget and refer you, if requested, to a move manager to make the transition a smooth one
  • Assist with the selection of and admission process to a nursing home

When indicated, you can work with a Board-Certified Patient Advocate, to easily access the care you need for your loved one that will address their individual needs.  Our mission is for our clients to enjoy fuller health and a quality of life while YOU as the concerned and loved one get to BE the loved one as WE get the health care in place.  Providing you with the answers to your care needs is at the heart of everything we do.

Medicaid

The process of obtaining Medicaid funded care can be lengthy and complex. We will outline the steps we need to take to achieve the best outcome.

For Community based Medicaid, we will:

  • Guide you as you collect required documentation
  • Prepare a Pooled Income Trust to shelter surplus income, if needed
  • Prepare and file a complete application
  • Coordinate home care and managed long term care (MLTC) services

For Long Term Nursing Home based Medicaid, we will:

  • Guide you through the admission process, including referral for a PRI (Patient Review Instrument)
  • Guide you as you collect required documentation
  • Maintain communication with the nursing home
  • Prepare and file a complete application

In all cases, we follow Medicaid applications through to completion.

Professional Background

Pam has a Master’s Degree in Gerontological Services Administration and is a Certified Senior Advisor.

She has enjoyed working with older adults for over 30 years in positions which have broadened her awareness and strengthened her desire to address their needs.
